Dive into five hospital-simulation games that range from slapstick medicine to detailed clinical management. These titles let players build facilities, hire staff and treat patients — sometimes fictional, sometimes eerily realistic — with distinct mechanics and tones.
Theme Hospital: the original comedic sim
Released by Bullfrog for PC in 1997, with a PlayStation port in 1998, Theme Hospital became a landmark for its cartoon visuals and sharp British humor. Creators Gary Carr and Mark Webley researched real hospitals before development, but the game leans into absurd, fictional ailments.
Players design rooms, recruit personnel and cure patients suffering from quirky conditions such as “Bloaty Head,” which requires a machine that literally inflates and then safely pops the patient’s head at the correct size. The title’s trademark is its comedic approach to hospital management.
Two Point Hospital: a spiritual successor in 3D
Two Point Hospital launched in 2018, published by SEGA, after Mark Webley and Gary Carr formed Two Point Studios in 2016. Set in the fictional Two Point County, the game modernizes Theme Hospital’s formula with full 3D graphics and expanded administrative systems.
Gameplay includes timed emergency patients, natural disasters and objective-based goals that award stars. One example: a clown emergency where patients must first see a psychiatrist and then pass through a circus-like machine to be “normalized.” The game preserves the original’s humor while adding deeper management content.
Project Hospital: clinical detail and complexity
Developed by Czech studio Oxymoron Games and released on October 30, 2018, Project Hospital emphasizes medical realism. Players design hospital layouts, hire staff, order tests, reach diagnoses and deal with insurance-driven cases. Hospital reputation unlocks additional departments.
The game’s focus on conditions grounded in real cases contributed to strong reception on Steam: an 88% positive rating across more than 8,800 reviews. Its strengths include depth and realism; its trade-offs are a steep learning curve and a dense user interface.
Galacticare: managing a health network across the stars
Brightrock Games from Brighton released Galacticare on May 23, 2024, for PC, Xbox Series X/S and PlayStation 5. In this sci-fi management sim, you serve as Director of an intergalactic healthcare network treating 49 conditions affecting multiple alien species.
Each species has unique needs, doctors possess quirks, and consultants follow skill trees. The game’s standout is its colorful, bizarre science-fiction twist on hospital management.
Surgeon Simulator: chaotic tabletop surgery
Originally created in 48 hours by Bossa Studios team members during the 2013 Global Game Jam, Surgeon Simulator’s expanded version arrived on Steam in April 2013. Played from a first-person perspective, you control a surgeon’s hand using mouse and keyboard to manipulate awkward tools and perform transplants.
The intentionally clumsy controls and lack of medical realism define the experience. The chaotic approach proved commercially successful: Surgeon Simulator sold 2 million copies by February 2015.
Which shift will you take?
These five games offer distinct approaches: comedic management, modernized spiritual succession, realistic clinical simulation, extraterrestrial healthcare and deliberately frantic surgery. Pick the style that fits your taste and step into the white coat — then tell others which hospital you’ll run first.
